Financial cushioning for chronic medication
Sitting within the Core or Insured Benefit Fund, the Generation Health Chronic Care Network (CCN) is designed to cater for members who need to manage chronic conditions such as diabetes mellitus, asthma, high blood pressure and HIV and AIDS to name a few. The CCN offers comprehensive cover with generous annual sub-limits per registered beneficiary.
Members with chronic conditions have to register on the CCN by completing an application form, which their doctor also completes, and the member then submits the form to the Fund. On receipt, the application is clinically assessed and if approved, the member is registered on the CCN for a specified duration and issued with a registration letter. It should be noted that the duration of the CCN registration is determined by the period covered by the prescription issued by the doctor. This period is stipulated in the registration letter and it is the member's responsibility to renew their CCN membership before it expires. Maintaining current membership on the CCN enables the Fund to pay for chronic medication from Insured Benefits and also ensures that the member is optimizing on their benefits.
The CCN letter can be used to obtain chronic medication from Pharmacies on Direct Payment with the Society.
